Instructions |

To download a document
- Make sure you have a copy of the free software Acrobat Reader installed,
otherwise, you can obtain one here.
- Once you have installed the software on your computer, click on the document you wish to download. The document should open in a new window.

To complete a form
- Once your .PDF file is open, you will have to save the file in order to keep a copy. This can be done using the "save" command in Acrobat Reader (do not confuse it with your web browser's save command).
- Print your document (check and adjust your printer for page setup and size).
- Complete your form and mail it back to the address mentioned on the form, along with any required documents.
