Instructions

To download a document Make sure you have a copy of the free software Acrobat Reader installed, otherwise, you can obtain one here. Once you have installed the software on your computer, click on the document you wish to download. The document should open in a new window. To complete a form Once your .PDF file is open, you will have to save the file in order to keep a copy. This can be done using the "save" command in Acrobat Reader (do not confuse it with your web browser's save command). Print your document (check and adjust your printer for page setup and size). Complete your form and mail it back to the address mentioned on the form, along with any required documents.
